You will be asked to sign a consent form for your procedure, which acknowledges your permission for the physician and facility to care for you. If the patient is a minor, this form must be signed by a parent or legal guardian.
If you have not completed your medical history on this webpage prior to the day of surgery, please call an intake nurse at (817) 898-6610 to complete it by phone or arrive 30 minutes earlier than scheduled. All medical histories are reviewed to assure your safety. We cannot request your history from your doctor.
